Carmella brings 5 of experience in care coordination, and is passionate about making a difference as CalAIM Program Manager at Hope Cooperative. In this role, Carmella guides a team in providing intense client management to Sacramento’s most vulnerable populations. Before joining the team, Carmella worked at WellSpace Health where they excelled from a direct caseworker to the role of Enhanced Care Manager Supervisor.
