Mental Health Case Manager

Multiple locations available: Personal Services Coordinator (Mental Health Case Manager) in Sacramento and Yolo County. This position assists clients with financial matters, entitlement issues. and seeks to instill a sense of responsibility, and independence.

Outreach Specialist

The Outreach Specialist will work with a multi-disciplinary team consisting of the City of Sacramento Community Response Team, Hope Cooperative, and other providers and partners as required.  This outreach team will conduct outreach to homeless individuals and families primarily located in the downtown Sacramento and River District areas and connect them with needed services.

Mental Health Clinician

The Mental Health Clinician is responsible for clinical assessment, creation and or oversight of a service plan, and implementation of that plan through therapeutic interventions. Positions available in both Woodland and Sacramento.

HUD Specialist

The HUD Specialist has the responsibility of completing tenant records and related occupancy documentation required by the agency Rental Assistance Programs, Leased Housing, SHRA and HUD (per related HUD guidelines) for all Rental Assistance Housing.
