Multiple locations available: Personal Services Coordinator (Mental Health Case Manager) in Sacramento County. This position assists clients with financial matters, entitlement issues. and seeks to instill a sense of responsibility, and independence.
The Clinical Manager is responsible for the day to day operations and clinical oversight of clinical care.
The Residential Case Manager works as part of a team to provide and coordinate services and resources for clients transitioning from homelessness into permanent supportive housing.
The Outreach Specialist will conduct outreach to homeless individuals and families primarily located in the downtown Sacramento and River District areas and connect them with needed services.
